Required Electronic Forms
All graduate students* registered for 6 credits or more regardless of degree program or matriculation status are required to complete the following health form requirements located in the student health portal.
GRADUATE STUDENT MENINGOCOCCAL VACCINATION RESPONSE FORM
- ALL graduate students are required to complete the Graduate Meningococcal Response Form.

Siena UNIVERSITY Immunization Record
- To provide proof of immunity to measles, mumps, rubella, meningitis, tetanus/diphtheria/ pertussis.
- ALL full-time, part-time, and non-matriculated students registered for 6 credits or more, born after January 1, 1957.
Required Document - upload a signed, stamped, and dated official immunization record prepared by a healthcare provider.
Any requests for medical or religious exemptions related to the University’s required immunizations must be submitted by June 30th at 5:00 p.m. for Fall Semester and prior to the start of classes for Spring Semester. Submissions after this date and time will not be accepted.
Tuberculosis (TB) Screening Questionnaire
- ALL full-time, part-time, and non-matriculated students registered for 6 credits or more.
Students must complete the screening questionnaire prior to the start of class.
* Note: Unless living or working on campus, graduate students enrolled in the following fully online programs are exempt from submitting health forms:
- Master of Science in Early Childhood & Childhood Education (MSEd)
- Master of Arts in Curriculum & Instruction (MA C&I)
- All TESOL or ENL-related programs
