Please review the How to Register Guide and below instructions for details on how to complete the registration for accommodations.

1.  Complete the following Registration Form and Authorization for Release of Information Form in full and return them to the Office of Accessibility.  You may complete the online forms using the links below or download, print, fill out the paper forms and send to Accessibility. For the online forms please use your Siena credentials (without @siena.edu) and Siena Password.

2.  Provide recent documentation of your disability which details your the major life function affected and accommodation needs. Documentation should be sent to The Office of Accessibility; this can sent directly to the Director of Accessibility or The Office of Accessibility.  

This must be generated by a properly credentialed professional. This could be a High School Documents such as a 504 Plan, an IEP 

  • It should provide information which establishes a disability
  • describes the functional limitations of the disability
  • how it inhibits a major life function
  • suggests reasonable accommodations

3.  Complete an intake meeting with the Director of Accessibility. Each student will be met with an interactive individual process. Please coordinate your availability with the Director for this short meeting to occur.  

4. Once registered, you will receive your accommodation letter for the current semester. 

A new accommodation letter is required every semester; you will request your accommodation letter via a form sent before the start of the semester. You then will be emailed your accommodation letter. This email will also be addressed to your instructors that you are registered for at the start of the semester. If you add any courses, it will be your responsibility to deliver your letter to the new instructors. If you are registering after the semester has begun, the Office will email your letter to you and your instructors upon completing your registration. 

Siena University makes every effort to follow AHEAD's most recent guidelines regarding disability documentation. Documentation for accommodations must be provided from an off campus provider. Siena's Health Services and Counseling Center may provide guidance for how to pursue obtaining documentation but will not be providing documentation for accommodations through the Office of Accessibility.
