Machine Learning Models in Primary Care Decision Support

Dan Loman '14 and Francesca Romano '14, Allied Health

A major interest of primary care provider groups is in identifying patients who are at risk for complex disease burden, adverse events, or high costs.

For example, which patients are at risk for a fall resulting in serious injury? Or which patients with a recent hospital admission are likely to be readmitted within 30 days of discharge?

Machine learning models trained to answer questions like these can supplement healthcare providers, nurses, and care managers’ workflows to help identify and outreach to patients most at risk.
