1. Complete the forms found here. Return them to the SSD Office.
2. Provide recent documentation of your disability which details your functional limitations and accommodation needs. Documentation should be forwarded from the student's high school, certified psychologist, licensed or otherwise properly credentialed professional.
3. Schedule an initial meeting with the Director to discuss your needs and accommodation requests.
Once registration with the Office is complete, if accommodations are granted, the student will receive a Confidential Letter of Accommodations which he or she will be responsible for distributing to faculty.
