Description of Outstanding Documents for Financial Aid
Please submit your outstanding requirements via email to aid@siena.edu, or fax to 518-783-2410 or mail to : Financial Aid Office, 515 Loudon Road, Loudonville, NY 12211. Some outstanding requirements do not require a copy/form to be sent to our office. Stafford and PLUS MPN records, Stafford Entrance and Exit Counseling and Perkins Entrance and Exit Counseling will automatically be submitted to the college electronically once completed.

Completed Consortium Agreement
Copy of DD-214 Form (Certificate of Release/Discharge)
Please submit a copy of your DD-214 Form to the Office of Financial Aid.
Copy of Driver's License
Please submit a copy of your (the student's) driver's license to the Office of Financial Aid.
Copy of Parent's 2011 W-2
Please submit a copy of your parent's 2011 W-2. If your parents were not required to file a federal tax return in 2011, please check the appropriate non-filer box on your Verification Worksheet.
Copy of Social Security Card
The SSN that you provided on your FAFSA does not match the SSN on record with the federal government. The Financial Aid Office is required to confirm this information before your aid is processed. Please submit a copy of your Social Security card to the Office of Financial Aid.
Copy of Father/Step Father's Social Security Card
The SSN that you provided for your father/step father on your FAFSA does not match the SSN on record with the federal government. The Financial Aid Office is required to confirm this information before your aid is processed. Please submit a copy of your father/step father's Social Security card to the Office of Financial Aid.

Please submit a signed copy of your parent's 2011 New York state tax return.

Please submit a copy of your selective service registration to the Office of Financial Aid. If you have not registered with the selective service, please do so at

Direct Loan Entrance Counseling (Stafford)**

www.studentloans.gov. Your Direct Stafford loan will not be processed until your Entrance interview is complete. For step by step instructions, please visit

Documentation Indicating Value of Assets
Please print and submit the
Asset Verification form using information from your 2011 Federal tax return.
Documentation of Child Support Received/Paid
Documentation of the child support received/paid for your household must be submitted to the Financial Aid Office to complete your financial aid.
Electronic Record of FAFSA Filing
Please submit the FAFSA online at
www.fafsa.gov and complete an electronic application for the 2012-2013 school year. A record of your completed FAFSA will be forward to Siena College. If you do not plan to file a FAFSA, please email
aid@siena.edu and indicate you will not be filing.
Federal Tax Return Transcript 2011 - Parent
Please submit a 2011 Federal Tax Return Transcript from the IRS. Your Free Federal Tax Return Transcript can be obtained from the IRS by:
- Calling the IRS Transcript order line at 1-800-908-9946.
- Submitting your request for a transcript online at the IRS website.
- Completing the paper Form 4506-T with the IRS.

PLUS Borrower Information Form
Please print and complete the PLUS Borrower Information form so we may process your PLUS loan for your Undergraduate Student.
Proof of Citizenship
You must submit proof of your United States citizenship or permanent resident status. This includes but is not limited to a copy of Certificate of Naturalization, U.S. Birth Certificate Current Passport and/or Alien Registration Card.
Request for Reinstatement of Federal Financial Aid
Please submit a letter to the Financial Aid Office via email to
aid@siena.edu, appealing the removal of your federal aid. Your letter should outline the special or extenuating circumstances that prevented you from meeting the academic requirement, the action plan you will undertake to achieve better grades, and any other information about your situation that you wish to be considered.
Request for Reinstatement of Merit Aid
Please submit a letter to the Financial Aid Office via email to
aid@siena.edu, appealing the removal of your merit scholarship. Your letter should outline the special or extenuating circumstances that prevented you from meeting the academic requirement, the action plan you will undertake to achieve better grades, and any other information about your situation that you wish to be considered.
Request for Reinstatement of State Aid
Please submit a letter to the Financial Aid Office via email to
aid@siena.edu, appealing the removal of your state aid. Your letter should outline the special or extenuating circumstances that prevented you from meeting the academic requirement, the action plan you will undertake to achieve better grades, and any other information about your situation that you wish to be considered. Signed Copy of Financial Aid Award Letter
Please sign and return a copy of your financial aid award letter. Your aid will not be processed until your award letter has been signed and returned to the Financial Aid Office. If you do not have a copy, please contact the Financial Aid Office at 888-287-4362 or e-mail
aid@siena.edu to request a copy of this letter.
Signed Worksheet for Tax Non-Filer
If you or your parents were not required to file a federal tax return in 2011, please check the appropriate non-filer box on your Verification Worksheet. Sign the document and return it to the Financial Aid office.
Stafford Exit Counseling
Please complete your Stafford Loan Exit Counseling at
www.nslds.ed.gov. Once at the site, select the "Exit Counseling" box and follow the instructions provided on the website. You will need for your Federal FAFSA PIN to complete your exit counseling session. Federal regulations require that all students complete an exit counseling session for their Stafford loan when they leave school. Please complete your TEACH Agreement to Serve at
https://teach-ats.ed.gov. Once you arrive at the site, select the My TEACH Grant and follow the step by step instructions provided on the website.
