Request for Vacation Housing Form
Please complete all of the requested information below to apply for vacation housing. All submissions will require additional verification from your coach, supervisor, faculty member, or staff liaison; these should be emailed to firstname.lastname@example.org.

Important information about Vacation Housing:
Vacation Housing Request Forms are due per the schedule on the Housing Information page. Late forms and/or verification may be charged a $25.00 late processing fee and/or denied authorization.
Students will only have access to their respective residence hall or townhouse.
All students applying must provide verification of their reason requesting vacation housing.
All Siena Life policies/procedures are in effect during vacation periods. In addition no guest/visitors (this includes current students who are not authorized for vacation housing) are allowed. Students found violating any Siena Life policy will be charged through the student code of conduct system and/or may be removed from vacation housing immediately.
It is critical for the safety and security of all students approved for vacation housing that students not provide access to the residence hall to any other student/individual. This includes, but is not limited to, holding/propping the entrances open. Providing such access is a safety concern, a violation of housing policy resulting in a referral to the student conduct system and/or removal from vacation housing immediately.
Approval or denial of a request will be based on the reason for the request, student code of conduct history, and facility availability during the break period.
There will be no meal service provided over the break period. Students will be responsible for all meals.
The only students permitted to remain in housing during the vacation housing period are:
In-season NCAA athletes required to attend practice/competition held outside the housing contract period;
Students currently involved in a credit-bearing internship program;
Unpaid student volunteers working in service to the College;
Students who have housing as a requirement of their employment per IRS tax rules.
Students employed by a college office (a minimum and/or equivalent of 40 hours a week during the break period.
EXCEPTION FOR END OF SPRING SEMESTER HOUSING:
Students who are requesting housing between the conclusion of their final exams and Commencement/begining of summer hosuing may only request for the following reasons:
Working as a commencement usher as approved from the Office of Academic Affairs, Chaplin's Office, or ROTC
Junior Class Office as approved by the Office of Student Activities and Leadership Developoment
In-season student athlete as approved by the Office of Athletics
Requesting extended housing due to enrollment in Summer Session I courses (all other summer residents will move in on May 25th).
Deadlines to request vacation housing are as follows:
Semester Break – requests must be submitted by December 6, 2013
Spring Break – requests must be submitted by February 21, 2014
Easter Break - requests must be submitted by April 2, 2014
End of Spring/Commencement housing - requests must be submitted by May 1, 2014
